Medication Assistance Program
Various options are available through medication assistance programs from pharmaceutical companies, medication samples donated to CHCBH, and the United Way Voucher program. MAP staff will assist you with determining which option may be available to you.
Please bring the following information to your MAP appointment, so your eligibility for various programs can be determined:
- If you work, your most recent pay stub
- If you receive social security, a current benefit statement
- Your latest tax return
- If you do not have a tax return, you will be provided with a 4506T Tax form to complete
Please ask your provider about the Medication Assistance Program. MAP appointments can be made at the front desk or by calling for appointments.
