Health Care for the Homeless/Outreach Locations

Are you experiencing homelessness? If so, you are not alone. The Community Health Center of the Black Hills Health Care for the Homeless (HCH) can provide the care you need. HCH serves people who lack stable housing. This includes people living in shelters, transitional housing, on the streets, or “doubled up” with friends, neighbors, or relatives.

Homeless Outreach Program

We offer assistance to adults and children that are homeless.

  • Nursing Assessment to Determine Health Needs
  • Medical Referrals
  • Case Management Services
  • Health Education and Awareness
  • Mental Health
  • Oral Health (By Referral to Oral Health Center)
  • Medication Assistance Program (By Referral to Health Center)
  • Substance Abuse Detoxification Referral
  • Referrals to other services including Housing Access, Nutritional Needs, Protective Payee
  • Entitlement Eligibility Assistance
  • Immunizations
  • Sexual and Reproductive Health Program (By Referral to Health Center)
  • Laboratory (By Referral to Health Center)
  • X-ray (By Referral to Health Center)
