Community Health Program

The Community Health Center of the Black Hills (CHCBH) recognizes the health care needs of all citizens of Rapid City, Pennington County and surrounding counties. CHCBH specializes in providing the highest quality health care.

Services Provided

Adult Medicine

Our Family Practice physicians, physicians' assistants, nurse practitioners, and nurses work together every day to provide you with compassionate, quality care. We offer physicals, immunizations, preventive medicine, same day sick visits and chronic disease management.

Pediatrics

From newborns to teenagers, your children are in the best of hands with our Pediatric team. We offer same day sick visits, well child checkups, physicals and immunizations.

Well Women Exams

We offer gynecological screening for breast cancer, cervical cancer and colposcopies.

HIV/STI Testing

We offer HIV and STI testing with Jennifer Sobolik, CNP, who received her HIV Specialist Credentials from the American Academy of HIV Medicine.

We also provide testing and treatment for the following STI’s:

  • Syphilis
  • Chlamydia
  • Gonorrhea
  • Trichomonas
  • Herpes type 1 and 2
  • Hepatitis B Screening
  • Hepatitis C Screening
  • Pap Smear (used for HPV in women)

Our patients have access to high quality care thanks to our growing list of specialty providers who graciously volunteer their time at the Community Health Center.

Marketplace Outreach and Enrollment

This program offers FREE, in person assistance with our Certified Application Counselors for the New Health Insurance Marketplace. This program provides assistance to individuals and families interested in the Health Insurance Marketplace under the new Affordable Care Act

Entitlement Eligibility Assistance

If you don't have insurance, our staff can help. We can help you locate any entitlement or assistance programs for which you or your family may be eligible.
