To schedule an appointment call (605) 721-8939. Please let our Patient Support Specialist (PSS) know which LOCATION you would like to be seen at. You should plan to arrive at least 15 minutes before your scheduled appointment.
For routine appointments, please schedule them with as much advance notice as possible. While we do offer some urgent care same-day appointments, it is important that you call ahead to schedule your appointment.
To schedule a dental appointment call (605) 721-8919. We provide primary and preventive dental services including routine screenings and cleanings, fillings and other services all at a reasonable cost.
You should plan to arrive at least 15 minutes before your scheduled appointment. You may receive urgent dental care on a walk-in basis and you should sign-in during clinic hours. Walk-in patients are treated on a first-come, first-served basis and space may be limited.
Because of the high demand for our dental services, we are only able to offer ongoing care to adults and children with Medicaid or Private Insurance. Urgent care will be available for low income and uninsured individuals at a discounted rate based upon family size and income level.

We accept Medicaid, Medicare, and private insurance. Please check with the Health Center for details.
Medicaid and private insurance are accepted. Medicare does not pay for dental services. Note: Please contact your insurance carrier directly if you have any question on your coverage. You are responsible for any non-covered services.
Patients who do not have health insurance may be seen as self-pay patients through our CHCBH Discount Program. Self-pay patients will be charged for services based on their income and family size. Patients should bring proof of income (for the last 30 days) to their first appointment.
Income is due within 10 business days and can be emailed to patientsupportstaff@chcbh.org. By signing the Discount Application you are confirming that all information shown is correct. And you understand the requirements for Proof of Income.

There are many programs available through CHCBH to assist self-pay patients and those with high insurance deductibles to obtain affordable health care. Please ask a CHCBH staff member for more information.
Community Health Center of the Black Hills does not refuse service to anyone based on ability to pay. Please call or visit one of our LOCATIONS for information.

You can call or stop in to schedule an appointment. To make a change in your appointment time please call 24 hours ahead or as soon as possible.
If you are late for an appointment you may be asked to wait past the scheduled appointment time or the appointment may need to be rescheduled.
If an appointment is missed and sufficient notice isn't given, the appointment will be considered a No Show. After one No Show, patients will be asked to call or come into the clinic for a Same Day appointment. An appointment following a No Show will not be scheduled in advance.
An On-Call provider is available after hours of operation at (605)721-8939 option:9. This service is available for urgent needs that cannot wait until the next day. If your question or concern can wait until normal business hours, you’re encouraged to call at that time. For emergencies dial 9-1-1.
Yes, you can still receive dental care if you are not a CHCBH medical patient. Our Oral Health Center accepts Medicaid and private insurance for adults and children. All dental services are available for our Flat-Fee Discount Program to income eligible, uninsured residents.
