Outpatient Counselor

As part of an integrated health model at the Community Health Center of the Black Hills (CHCBH), the Outpatient Counselor is the primary interface for patients needing behavioral health services and/or opioid use or abuse services. This individual will provide screening, assessment, individual and group counseling, education, and case management services for CHCBH patients.
Essential Function:
- Based on Medical Provider referrals will conduct brief, non-stigmatizing screenings to evaluate needs of adults, adolescents, and/or children for mental health and/or opioid abuse services based on medical record, direct interview and interaction and/or discussion with other staff members who interact with patient.
- Perform comprehensive bio-psychosocial assessment for mental health, substance abuse, co-occurring disorders, domestic violence and medical needs. Conduct follow-up as appropriate based on results of evaluation.
- Develop, implement, and evaluate plans for the provision of mental health and support services jointly with clinic staff.
- Establish and maintain a current listing of community linkages to meet the needs of the target population. Develop and maintain collaborative relationships with community providers of services. Initate linkages, disseminate information and make referrals to the appropriate services as needed.
- Use an inter-agency team approach when working with community partners to ensure referrals are appropriate and meet the patients’ needs.
- Develop, implement, and evaluate a case service plan (in addition to the bio-psychosocial assessment) including agreed-upon goals to address patient needs including social supports to promote long-term self-sufficiency.
- Provide early interventions and referrals to specialty mental health services, including referral for more intensive psychological or psychiatric support services, if needed.
- Document findings in patient’s medical records according to established protocols.
- Initiate and take on special responsibilities as required by grant funded projects and other program development efforts.
- Provide ongoing consultation for primary care staff in appropriate mental health techniques.
- Participate in ongoing training in behavioral health issues and provide training to primary care clinic staff in mental health topics including counseling skills and confidentiality, medication use and misuse, substance abuse, wellness, rehabilitation, recovery and self-sufficiency.
Working Environment:
- Clinic environment. Weekdays. May involve occasional after hours, weekends.
Education & Experience:
- Certified Social Worker or Licensed Professional Counselor required; CSW-PIP or LPC-MH preferred
- Currently licensed with the state of South Dakota
- Minimum experience between 2-3 years
- Previous experience working with ethnically and culturally diverse families
